Student Packets

• Complete the Student Packet Form using JotForm.
• Upload vaccine record/insurance card to the form.
• Mail the student fee and photo to the director by 5/25/2024.
• Complete the Participant Survey by 5/31/2024. The password is in the welcome email from the direcor.
